Preparing for Your Appointment

Please follow these steps to prepare for your first appointment with us.

Patient Portal

Though not required, we highly recommend that you register for and utilize our patient portal, which has many useful features:

  • Secure messaging

  • View, download, send reports and test results

  • Share documents and files with us

  • Manage appointments (limited online scheduling available)

  • Complete, review, and electronically sign forms

  • Online check-in to streamline the registration process on the day of your appointment

  • View statements and pay balances

  • Download educational materials

Text Messaging

Feel free to text us at (908) 836-7448. Please keep in mind that standard texting (SMS) is not a secure form of communication and confidential medical information should be avoided when communicating through text.

We recommend using our HIPAA-compliant, secure texting service that is available through the Spruce app which you can find here or scan the QR code below with your smartphone:

Forms

Please complete this questionnaire and return it to our office at least one week before your appointment. This will allow the time dedicated for your visit to be the most effective. It is a “fillable” form in Microsoft Word. It can also be printed, and completed my hand as well. Thank you.

If possible, please complete these forms before arriving for your appointment.

    *Available for electronic signature through online check-in

There is one additional form (Patient Registration and Acknowledgement Form) that needs to be completed and signed with pen when you arrive at the office.

Referrals

It is the patient’s responsibility to know your insurance plan’s requirements and obtain necessary referrals/authorizations. Please obtain all referrals/authorizations prior to your visit to avoid cancelations or charges.

Medical Records

Please request medical records to be faxed to us at (833) 974-2499 or mailed to:

They can also be emailed as PDFs to info@peds.rehab. Physiatry, Orthopedics, Neurology, Neurosurgery, and Therapy reports tend to be particularly helpful. Other documents that may be helpful include:

  • Individualized Education Plan (IEP) or 504 Plan documentation

  • X-Ray reports or films, particularly of pelvis and spine

  • MRI reports, particularly brain and spinal cord

  • Genetic Testing reports

  • Surgery Reports

Other Things to Bring

Please bring your insurance card and official ID (e.g. driver’s license, government-issued ID card, passport). A good joke or riddle would be a bonus. Dr. Cheng loves jokes and riddles.

What to Wear

Please dress in loose, comfortable clothing such as shorts or sweatpants. Also, please bring any braces/splints that you use. If you utilize braces for your feet/ankles, please be sure to bring the shoes that you wear with those braces.

COVID-19 Policy

Masks are encouraged at this time.
